Continuity of care beyond discharge: residential and outpatient therapy, fitness, and nutrition in one center, with connectivity to healthcare professionals through the MCHUSA digital network.

Overview

Hospitals have traditionally offered rehabilitation services to patients during their stay. However, many individuals discharged from hospital care continue to require ongoing physical or occupational therapy. Locating an appropriate rehabilitation center to provide these essential services can be a complex process.

Furthermore, individuals who have completed rehabilitation programs often seek access to gym facilities, while others may pursue gym activities to promote physical health and overall wellness. Although outpatient rehabilitation facilities and numerous gyms are available in Georgetown, there is a recognized need for similar programs in various regions where such resources are comparatively limited.

To meet these needs, MCHUSA proposes the development of a Rehabilitation, Gym & Nutrition Center in various regions of Guyana. This facility will provide residential and outpatient therapy services for individuals discharged from hospitals and will also serve those interested in gym amenities. Persons can use the network connectivity to make their medical information available to therapists and nutritionists.

Center Structure

The Center will occupy a four-story building with 16 beds in double-occupancy rooms with bathrooms on the two upper floors. An elevator will serve all floors, and stairlifts will help residents move between their floors and other areas.

The second floor will feature an open layout for physical and occupational therapy, gym activities, and outdoor use. The lower floor will house the kitchen, laundry, housekeeping, administrative office, and a sales area for nutritious foods. Offsite activities will also be offered at locations that support exercise and rehabilitation.

Residential and outpatient therapy areas at the MCHUSA Rehabilitation, Gym & Nutrition Center

Residential Services

Offered to persons staying at the Center, covering therapy and nutritional meals, ideal for those discharged from hospitals who require continuation of care. Residents may continue receiving services after discharge. A nutrition service provides meals for residents and a complementary package for gym members.

External Services

Designed for individuals enrolled in daily exercise programs and aqua therapy sessions, including access to the pool. Participants may also engage in activities in conformity with their therapy regimen. Elderly persons can be enrolled in water sport activities conducted at certain locations.

Gym & Health Monitoring

Gym activities include health status monitoring. Persons can have their vitals checked via a machine and a healthcare professional on site. They receive an assessment and recommendation regarding the type of fitness exercise they should pursue, together with food supplements that will enhance their health condition.

Offsite Exercise Program

Designed for elderly church groups and held at church premises. Each three-hour session includes chair exercises, games, music, and entertainment. Members receive transportation to and from the venue, along with snacks suited to their health needs.

Water Sports Activities

Elderly persons can be enrolled in water sports activities, during which they can enjoy canoeing accompanied by experienced swim instructors. This can be combined with a day program consisting of exercises, entertainment, and food.

Nutrition Services

The Center provides meals for residents and a complementary package for gym members. Members and residents may also purchase meals and food items. A nutrition specialist guides dietary choices aligned with each person's health condition.

Membership

Individuals may register for external center membership through the MCHUSA website Healthcare Network. Upon payment for their chosen membership category, they receive an access code for entry to the Center's 1st and 2nd floors (upper floors are restricted to residents and staff).

Members can use their login credentials to access additional healthcare services on the MCHUSA Portal without re-registration. Those who already have login access for other services are also eligible to access the Center.

Persons visiting the Center enjoy a complimentary nutritious snack and free blood pressure screening.

Session Structure

  • Eight (8) one-hour sessions per month
  • Physical therapy segments in 15-minute periods based on physiatrist assessment

Retreat Program

Participants can register for a Retreat Program offsite in either a rustic or traditional environment, combining rehabilitation and leisure in a supported setting.

Sponsorship Opportunities

Central Government

This program aligns with the Guyana Government's regional healthcare initiative. Centers could be located alongside regional hospitals, with funding from government service payments or through the Guyana National Insurance Scheme (NIS).

Regional Administration & Churches

Regional Administrations and churches can sponsor the offsite program, utilizing available spaces in their buildings. Programs can be offered twice weekly for four-hour durations, with transportation provided for the geriatric population and rehabilitation patients.

Private Sector

Private organizations may sponsor memberships or subsidize services for specific populations, contributing to healthier communities while meeting corporate social responsibility objectives.
